MRG003 (Becotatug Vedotin) + Sintilimab + Chemoradiotherapy in Locoregionally Advanced Nasopharyngeal Carcinoma
A multicenter randomized Phase 3 trial (First Affiliated Hospital of Guangxi Medical University) adding becotatug vedotin (EGFR-directed ADC) + sintilimab (anti-PD-1) to standard chemoradiotherapy in curative-intent, locoregionally advanced NPC.
Evidence: Emerging (backbone-supported, no same-setting readout)
Phase 3, N = 266, randomized, recruiting since March 2026, primary endpoint event-free survival (EFS). This trial moves MRG003 out of its proven recurrent/metastatic (R/M) salvage setting and into curative-intent, locoregionally advanced disease alongside chemoradiotherapy. The drug's track record is strong but entirely in R/M: randomized backbone data vs chemotherapy (N = 173, ORR 30.2% vs 11.5%, mPFS HR 0.63; Ann Oncol 2026) and a combo Phase 1/2 cohort with anti-PD-1 (N = 31, ORR 71%; JCO 2026). There is no direct readout of MRG003 in the locoregionally advanced / curative-intent setting yet — this Phase 3 is the first. Primary completion expected April 2030.
Why “Emerging”: every published efficacy readout for becotatug vedotin (MRG003) is in recurrent / metastatic NPC — a salvage population. This trial tests a fundamentally different setting: curative-intent, locoregionally advanced disease, combined with chemoradiotherapy. The evidence below is the drug's R/M track record presented as the rationale; none of it directly demonstrates benefit (or the added toxicity profile with concurrent CRT) in the curative setting. Read it as the basis for the hypothesis this trial is testing, not as evidence the combination works here.

Approved · China R/M label only MRG003 (Meiyouheng) — first approval, October 2025Approved in China for R/M NPC after ≥2 lines of chemotherapy AND PD-1/PD-L1 failure — i.e. a late-line salvage label. The locoregionally advanced curative-intent use studied here is investigational and well outside the approved indication.
The combination partner (sintilimab)
-
Partner agent Sintilimab — an established anti-PD-1 antibodySintilimab is a widely used anti-PD-1 checkpoint inhibitor; PD-1 blockade combined with gemcitabine/cisplatin is an established approach in NPC. The specific MRG003 + sintilimab combination has no published readout — the pivotal MRG003 combo data above used a different anti-PD-1 (pucotenlimab). No efficacy numbers are asserted here for this specific pairing to avoid over-reading unpublished data.

How this strength score was computed
| Dimension | Score | Basis |
|---|---|---|
| Direct evidence (this setting) | None yet | No published readout of MRG003 in locoregionally advanced / curative-intent NPC, or of the MRG003 + sintilimab combination in any setting. This trial is the first. |
| Backbone drug evidence | Moderate–High | MRG003 has Level II randomized data (N=173, HR 0.63) + a combo Ph1/2 cohort (N=31, ORR 71%) + a China approval — but all in R/M salvage. |
| Setting transferability | Low | R/M salvage activity does not reliably predict curative-intent benefit; ADC + concurrent chemoradiotherapy also raises an untested additive-toxicity question. |
| Trial design | High | The trial itself is Phase 3, randomized, with a hard curative-intent endpoint (EFS). |
| Source quality | High | Backbone evidence is from J Clin Oncol / Ann Oncol / JAMA Oncol (top-tier peer-reviewed). |
| Overall | Emerging | Strong drug, strong trial design, but a setting leap with zero direct evidence — a high-risk / high-reward hypothesis test. |
Limitations of this evidence base:
- All MRG003 efficacy data is in recurrent/metastatic salvage NPC. There is no direct evidence for the locoregionally advanced, curative-intent setting this trial studies.
- The specific combination — MRG003 + sintilimab + chemoradiotherapy — has no published readout. The pivotal combo data used a different anti-PD-1 (pucotenlimab).
- Adding an EGFR-directed ADC to concurrent chemoradiotherapy introduces an untested additive-toxicity question (skin, mucosal, hematologic) in a curative population where tolerability matters more than in salvage.
- All underlying evidence is from Chinese centers; generalizability to non-endemic (Type I / Western) NPC is untested.
- EFS readout is years away (primary completion April 2030); no interim efficacy exists today.
