MRG003 (Becotatug Vedotin) + Anti-PD-1 Maintenance in Recurrent/Metastatic Nasopharyngeal Carcinoma
A randomized Phase 3 trial (Sun Yat-sen University) testing becotatug vedotin (EGFR-directed ADC) added to anti-PD-1 as maintenance therapy in R/M NPC — i.e. switching in the ADC to deepen/prolong disease control after initial therapy.
Evidence: Moderate (Emerging)
Phase 3, N = 86, randomized, recruiting since July 2026, primary endpoint progression-free survival (PFS). This trial keeps MRG003 in its proven recurrent/metastatic setting but tests a new strategy — maintenance. The direct support is strong for the regimen class: MRG003 + anti-PD-1 produced a 71% ORR in an R/M Phase 1/2 cohort (N = 31; JCO 2026), and MRG003 monotherapy beat chemotherapy in a randomized R/M trial (N = 173, mPFS HR 0.63; Ann Oncol 2026). However, no study has yet read out MRG003 specifically as maintenance, and this Phase 3 is small (N = 86). Primary completion expected July 2028.
Why “Moderate (Emerging)”: the setting (recurrent/metastatic NPC) and the regimen class (MRG003 + anti-PD-1) both have supporting R/M data below — so this is on firmer ground than extending the drug to a brand-new disease stage. What is not yet demonstrated is the specific maintenance strategy (switching the ADC in after initial control, measured by PFS). No maintenance readout exists, the pivotal combo used a named anti-PD-1 (pucotenlimab) rather than the generic “anti-PD-1” here, and N = 86 is small for a Phase 3.

How this strength score was computed
| Dimension | Score | Basis |
|---|---|---|
| Direct evidence (maintenance strategy) | None yet | No readout of MRG003 used as maintenance. The supporting data is active-treatment R/M. |
| Setting match | High | Recurrent/metastatic NPC — exactly where MRG003 is validated and approved. |
| Regimen-class evidence | Moderate–High | MRG003 + anti-PD-1 gave 71% ORR / 12 mo PFS in an R/M Ph1/2 cohort (N=31); backbone randomized vs chemo (N=173, HR 0.63). |
| Sample size | Low–Moderate | This Phase 3 is small (N=86); the supporting combo cohort was also small (N=31). |
| Source quality | High | J Clin Oncol, Ann Oncol, JAMA Oncol + a first-approval review. |
| Overall | Moderate (Emerging) | Right setting, well-supported regimen class, but the maintenance strategy itself is unproven and the trial is small. |
Limitations of this evidence base:
- No study has read out MRG003 as maintenance therapy. All supporting data is active treatment in measurable R/M disease.
- The pivotal combo evidence used a specific anti-PD-1 (pucotenlimab); this trial specifies a generic “anti-PD-1”, so the exact partner may differ.
- N = 86 is small for a Phase 3 — adequate to detect a large PFS effect, underpowered for a modest one.
- Maintenance ADC exposure raises a cumulative-toxicity question (e.g. peripheral neuropathy with MMAE-class payloads) over prolonged dosing that short active-treatment readouts don't capture.
- All underlying evidence is from Chinese centers; generalizability to non-endemic (Type I / Western) NPC is untested.
